Job Description
Due to further contract award Precedo Healthcare are looking for Community Nurses to undertake Nursing Homecare in the Community in Leeds and surrounding towns/cities. Both full and part time positions are available and our contracts are ongoing.

Job Title: Nurse (Community)
Band: 5 or above
Service Users: Patients in the community– in the home, in clinics and in intermediate care
settings
Job Summary:
~ Works within codes of practice and professional guidelines. Work is managed rather than supervised.
~ Assesses patients, plans and implements care in the community
~ Provides advice and maintains associated records
~ Carries out nursing procedures
~ May provide clinical supervision to other staff
~ Works within an interdisciplinary team to deliver quality care with an emphasis on promoting independence and rehabilitation as well as curative and palliative care
Training/Qualifications:
Registered on the appropriate part of the NMC register for the assignment RN (Adult) / RN1/ RN2 OR RN (Child) / RN8
Experience:
Current (within last 6 months) evidence based practice within an NHS setting. If newly qualified (within the last year) - evidence that a preceptorship programme has been completed successfully
Skills & Abilities:
~ Able to assess, plan, implement & evaluate programmes of care
~ Good interpersonal skills with the ability to relate within the interdisciplinary team and across disciplines and agencies
~ Effective communication skills (written, non-verbal & verbal)
~ Able to function effectively as a team member
~ Self motivated, uses own initiative and will make decisions
~ Able to prioritise workload (self and others) and meet deadlines
~ Able to supervise and train junior staff
~ Maintains clear and accurate records of discussions, assessments made, treatment and medicines delivered and how effective these have been
~ Ensure all patients and relatives are treated with respect and dignity at all times
~ Competent and effective clinical nursing skills including:
- Administration of IV medicines
- Able to record an ECG
- Able to demonstrate safe use of patient equipment including infusion devices
- Competent in basic life support
- Effective wound care
